Responsibilities

• Assess physical, emotional, and social needs of geriatric clients
• Develop comprehensive care plans addressing health and aging needs
• Coordinate medical appointments, home care, and community resources
• Advocate for seniors and their families within the healthcare system
• Provide education and guidance to families and caregivers

Qualifications

Excellent organizational and follow-up abilities

Background in nursing, social work, or healthcare services

Experience working with geriatric populations preferred

Strong advocacy and communication skills
