Responsibilities
- Assess client needs and develop individualized case management plans
- Coordinate medical care with physicians, specialists, and healthcare providers
- Advocate for clients within the healthcare and insurance systems
- Monitor treatment progress and adjust care plans as needed
- Communicate clearly with clients, families, and care teams
- Maintain accurate and confidential case documentation
Qualifications
Ability to manage multiple cases effectively
Active RN or LPN license (required)
Experience in medical case management or care coordination preferred
Strong communication and organizational skills
Compassionate, client-centered approach
